Take advantage of FREE services, such as healthcare application assistance, free tax preparation, flu shots, dental screenings and other money saving programs and services!

FOR HEALTH CARE PROGRAMS AND/OR CALFRESH APPLICATION ASSISTANCE, Please bring these documents:
* Birth Certificate or Legal Resident Card for each person
* Income Verification for the last 30 days for all persons in the home
* Rent/Mortgage Statement (most current)
* Social Security Cards for each person

FOR TAX PREPARATION ASSISTANCE IF YOU MADE $50,000 OR LESS IN 2011, Please bring these documents and arrive by 2pm:
* 2010 tax return (if available)
* Proof of Identity (Sociaal Security Number or
* Individual Taxpayer Identi¬cation Number (ITIN) for each family member)
* Proof of income (Including W-2s and 1099 forms)
* Documentation of deductible expenses
* Account and routing numbers of checking and saving accounts for direct deposit and a faster refund
