It's hard to forget the shooting that scared this community. The same shooting that left two valley officers dead. And how so easily those images return to us with a report. There are more than one-hundred pages of detailed analysis. From the investigation into Rick Liles, to the shooting and even the aftermath.

The report was done by retired captains in Southern California. There were more than 87 findings and recommendations too. But Sheriff Margaret Mims wants you to know one thing.

"The blame of law enforcement deaths is solely with Rick Liles. It was his criminal activity that led to those incidents," said Mims.

The sheriff does admit Rick Liles' threat was over-looked. That's likely why Detective Wahlenmaier did not write up a risk assessment and that's likely why SWAT did not show to help serve the warrant.

"Before the incident there was clearly a lack of attention," said Sheriff Mims.

Some changes like in training have already been implemented. But the sheriff reminds us what her deputies have to do are in highly-stressful situations with seconds to act.

"I think the biggest lesson is we cannot take for granted the people who want to do us harm," said Mims.
